Dear colleagues and students, if you have traveled to China, Hong Kong, and Macao in the past two weeks, please fill in the following record form to perform self-management.

If you have any discomformt during these 14 days, kindly go to see the doctor with wearing mask and contact Health Section(02)27361661#2250 as soon as possible. If you do not have any discomfort during these 14 days, kindly screen your complete record form and send to health@tmu.edu.tw
As you have come back to Taiwan from China or Hong Kong or Macaw for 10 days, you should perform "self-health management" and record your temperature and health condition within the rest of 4 days.
